Emergency Hormonal Contraception (EHC)
Providing the Service
Lincolnshire County Council (LCC) have commissioned a service for the provision of free-to-patient emergency hormonal contraception to women aged 13-19 years. You must had an agreement in place with LCC as the commissioner, and have signed (and where appropriate returned) any required documentation, including the PGD before commencing provision.

Pharmacists providing the service must ensure that they have completed adequate training. Assessment of this is through a Declaration of Competence; you can find out more about the Declaration of Competence model at www.cppe.ac.uk/services/declaration-of-competence
Pharmacists must also ensure that that have up-to-date safeguarding training in place.

Payments will be made quarterly in arrears, based on the completion and submission of this claim form to Lincolnshire County Council by email (we suggest using your NHS mail account) to:

PLEASE NOTE THAT ANY SUBMISSIONS RECEIVED AFTER THE DEADLINES LISTED ABOVE WILL NOT BE PAID UNTIL THE FOLLOWING QUARTER.
